Toddler dies from playground injury at child care facility

A toddler has died after family members say her head got stuck in playground equipment at a child care facility in Wasilla.

The girl, identified as 19-month-old Faith Phillips, died Sunday. Family members told KTUU-TV that she had been hospitalized since Thursday morning.

The girl's mother -- 19-year-old Brittany Phillips -- said her child was playing outside Illuminations Child Care on playground equipment, when she stuck her head through a hole in the equipment while standing on a toy.

She says the girl then slipped and was strangled.

Phillips says there was a window in the playground equipment and her child's head fit right through the window.

The girl was flown to Providence Medical Center in Anchorage, where she was declared brain dead.

Inspectors from the state Department of Health and Human Services are investigating the death, said deputy commissioner Ree Sailors.

Illuminations Child Care's state license has been suspended while the investigation takes place, she said.
