LEGAL NOTICE - Effective February 7, 2013: Alaska Medicaid provider Faalele Namaulauti and Bethesda Assisted Living Home have been excluded from any and all participation in the Alaska Medicaid Program including and not limited to Denali KidCare, Assisted Living Home and the Home and Community Based Waiver program. Pursuant 42 CFR 1002 and section 1128(d) of the Social Security Act, the effect of this exclusion is that Faalele Namaulauti may not be hired or employed by any agency or entity that is currently enrolled with any state Medicaid program, Medicare, or any other federally funded program. This action is the result of non-compliance with State and federal regulations. Published: February 8-10, 2013
