LEGAL NOTICE - Effective February 7, 2013: Alaska Medicaid providers Maurice and Sandy Marulanda have been excluded from any and all participation in the Alaska Medicaid Program including and not limited to Denali KidCare and the Home and Community Based Waiver program. Pursuant 42 CFR 1002 and section 1128(d) of the Social Security Act, the effect of this exclusion is that Maurice and Sandy Marulanda may not be hired or employed by any agency or entity that is currently enrolled with any state Medicaid program, Medicare, or any other federally funded program.
